DR. TARR: If nothing there, we would try
antispasmodics. The treatments for emptying the
colon usually consist of some dietary changes,
but these are children we are treating and no
matter what dietary changes you recommend they
might not implement them. So we try to override
some of these diet problems with lactulose, or
Miralax. These are non absorbable substances
that help to suck water into the colon and help
flush stool out.
We suggest a trial of fruit
withdrawal; that can be difficult. We can
really impress people with the necessity for
doing a fruit withdrawal after we come back with
a breath hydrogen tests which is dramatically
positive. And often the child is symptomatic at
the end of the test.
This is an antispasmodic,
which is sometimes helpful.
And in very rare cases,
I've heard of this mostly in adults, mostly with adult gastroenterologists
calling me, people are left
with what seems to be clear cut motility
disorders where the gastrointestinal tract does
not work in a linear matter. Foods and
gastrointestinal contents do not go from fore to
aft in the right frequency. The gastrointestinal
tract has discordant contractions or there's no
contractions at all.
We try some promotility agents,
which Metaclopromide is now the only one
available. And this can be a very, very
frustrating disorder to deal with.
